Tax Exempt Number: Enter a tax exempt number if: (1) you are a valid reseller (2) an approved non-profit organization, (3) your business is outside of the state of Ohio or (4) your tax accountant advises you are exempt from state taxation under certain federal regulations.

Single sided or double sided: Make a selection based on the majority of your page makeup.

Number of pages w/o cover: A page constitutes a printed impression. For example, an 8.5 x 11 sheet of paper printed on both sides is two pages.

Paper Stock: Cover Info: If the cover of your booklet is made of the same paper stock as the rest of your booklet do not fill out this section. Your booklet is considered to have a self-cover. Only use this section if your cover page is a different paper stock than the rest of your piece.

Paper Stock: Guts Info: Use this section for (1) the paper stock of the inside pages of your booklet, manual, or binder and (2) the paper stock of your stand alone one-sheet, brochure, or spec sheet.
